         We are pleased that you are interested in Prairie Moor. We are dedicated to providing you with the finest in assisted living care.    
 
What is Prairie Moor?
 
•     Prairie Moor Adult Family Home has the capacity for four (4) residents. 
•     We provide services to ambulatory, developmentally disabled clients, both men and women.  We provide assistance in daily living activities, social activities for the residents, and security for the resident’s family.  We strive to meet all of our residents needs, whether they are physical, emotional, or social. 
 •     We will personalize each client’s ISP to meet the specific needs of that client, as each client typically has varied outcome goals.  Arbor Living will only accept clients who are compatible with existing clients, who are in a client group our staff is trained serve, and whose needs we are able to meet.   Safety for existing clients, staff and new clients is taken seriously, and is a primary consideration when accepting new clients.  We will develop and implement a plan to maintain, improve or exceed the skill level of each client through assistance and training in Activities of Daily Living and Community Living Skills.
 
 Where is Prairie Moor?
 
•     Prairie Moor AFH is in the city of Waukesha, Waukesha County, Wisconsin.  The home is in a nice part of town within walking distance of parks. It is near the Waukesha Memorial Hospital. 
•     This Duplex home has a Supported Apartment Program on one side and Prairie Moor AFH on the other. Prairie Moor has two (2) very large bedrooms, one full bath, one half bath, a large kitchen and dining room with a spacious living room.  The bedrooms are on the second floor, so residents will need to be able to use stairs daily. 
 
 Who is the staff at Prairie Moor?
 
•     Our staff is loving, caring and dedicated to providing residents with the care they need while encouraging independence.  They are happy people, enjoy life, love what they do.  All staff take continuing education classes to fulfill requirements, are trained in safety, and have passed a background check.   Arbor Living’s Administrator, Robin Gedney is available by phone any time, and is frequently on site at Prairie Moor.  If the administrator is not available for a period of time the house manager is on-call, contact information will be made available upon admission. 
 
 What do we do at Prairie Moor?
 
•     We provide three healthy meals daily with snacks.   Medications are ordered, monitored and administered properly by trained staff.   Our staff assist residents with meal preparation, ensuring a healthy diet of fresh fruits and vegetables, whole wheat products, and a minimal amount of fats and sugars. 
 •     The home has a van to transport residents.  We transport residents to any needed appointments.  Also to community functions, church, restaurants, bowling, art activities, work/day program and many activities they are interested in attending. 
  
 What is Arbor Living?
 
Arbor Living is a company that provides assisted living in a residential family like setting for disabled adults who do not require skilled nursing care. We provide 24-hour assisted living services by trained staff.  Arbor Living was created to provide an excellent place for our younger brother to live.  Our standards for each resident are set by the dedication we have to ensuring the best for our brother.  We are pleased to be able to provide this opportunity to other adults, who like all of us, want to live life to the fullest!
